When your hormones are off, everything is

Energy, sleep, mood, weight, libido, focus — they run on hormones, and when the balance drifts, all of them can too. In Fredericksburg, VA, we test the full picture and optimize precisely, for men and women. Start with an initial appointment.

One system behind many symptoms

Thyroid, sex hormones, insulin, and cortisol all talk to each other. When one is off, the effects ripple — which is why fatigue, weight gain, poor sleep, low mood, and low libido so often arrive together and rarely resolve when treated one at a time.

At True You, Dr. Hoff tests the full hormonal picture and builds one connected, physician-guided plan to bring the system back into balance.

Why “normal” isn’t the goal

Why one test at a time keeps missing it

The difference is testing the whole system — against optimal, not just “not flagged.”

The conventional visit

  • One hormone tested in isolation
  • “Normal range” = anything not flagged
  • Symptoms treated one at a time
  • A prescription and a shrug
  • A rushed visit

The True You approach

  • The full panel — thyroid, sex hormones, insulin, cortisol
  • Optimal range — how a balanced system actually reads
  • One connected plan for the whole picture
  • Root causes across metabolism, stress, and hormones
  • Unhurried time with a physician who monitors over time

A value can be “in range” yet far from optimal — which is why balanced-looking labs still leave so many people feeling off.

What causes hormone imbalance?

Some causes are part of life: perimenopause and menopause in women, and a gradual decline in testosterone in men. Others are driven by the way the hormonal system is connected. Chronic stress can disrupt cortisol rhythm, which affects sleep, blood sugar, and sex hormones. Insulin resistance can raise androgens in women and lower testosterone in men. Thyroid autoimmunity, such as Hashimoto’s, can slow everything down.

Sleep, body weight, nutrition, gut health, and certain medications play a role as well. Because the causes overlap, two people with the same symptoms can need completely different plans — which is why we test before we treat.

Hormone imbalance in women — and in men

In women, imbalance often shows up around cycles and life stages: heavy or irregular periods, PMS that’s getting worse, perimenopause sleep and mood changes, or the higher androgens and insulin resistance of PCOS. A slow thyroid can sit underneath any of these.

In men, it more often looks like fading energy, drive, and strength, more belly fat, and lower libido — frequently tied to low testosterone, insulin resistance, poor sleep, or stress. In both, the answer starts with testing the whole system rather than one hormone at a time.

How it works

How we diagnose and treat hormone imbalance

Connected testing, one connected plan.

  1. 01

    Step 1

    Initial appointment

An in-person visit at our Fredericksburg office to map your symptoms, history, and goals.

  2. 02

    Step 2

    Full hormone panel

Thyroid, estrogen, progesterone, testosterone, cortisol, and metabolic markers such as insulin.

  3. 03

    Step 3

    Find the driver

Dr. Hoff reviews how your results fit together and what’s most likely behind your symptoms.

  4. 04

    Step 4

    Your plan

Options may include bioidentical hormone therapy, thyroid support, metabolic care, and sleep and stress changes.

  5. 05

    Step 5

    Monitor over time

Repeat labs and follow-up — many visits virtual — so the plan keeps up with you.

Treating the whole system, not one number

When hormones are low, replacing them can help — and bioidentical hormone replacement therapy is often part of the plan. But if insulin resistance, a sluggish thyroid, or a disrupted stress response is driving the imbalance, those need attention too. Treating them together is what lets results last. When the picture is complex, our functional medicine work-up ties it together.

Who it’s for

A hormone work-up may help if you…

  • Have several symptoms at once — fatigue, weight, sleep, mood, libido
  • Were told your labs are normal, but you still feel off
  • Are in perimenopause, menopause, or noticing changes in your 40s or 50s
  • Are a man with fading energy, drive, or strength
  • Have PCOS, thyroid disease, or insulin resistance
  • Want one physician looking at the whole picture

Hormone imbalance — common questions

Which hormones do you test?
We look at the full picture — thyroid, sex hormones (estrogen, progesterone, testosterone), and metabolic markers like insulin and cortisol — because they influence each other.
Is this only for women?
No. Hormone imbalance affects men and women. We tailor testing and treatment to you, whether that’s perimenopause, low testosterone, thyroid, or a combination.
Do you take insurance?
True You is a cash-based practice, so your care isn’t limited by insurance rules. We’re transparent about costs during your consultation.
Can hormone imbalance cause weight gain?
Yes. Thyroid, insulin, cortisol, estrogen, and testosterone all influence appetite, metabolism, and where your body stores fat. Correcting the imbalance often makes weight loss easier.
Can hormone imbalance cause anxiety or low mood?
It can. Shifts in estrogen, progesterone, testosterone, thyroid hormone, and cortisol can all affect mood. We look at hormones as part of the picture, alongside the rest of your health.
Can stress cause a hormone imbalance?
Chronic stress affects cortisol, which in turn influences sleep, blood sugar, and sex hormones. Managing stress is often part of rebalancing the system.
Can hormones be balanced without hormone therapy?
Sometimes. When the driver is sleep, stress, weight, or insulin resistance, addressing those can improve hormone levels. When hormones are genuinely low, therapy may be the most effective option.
What kind of doctor treats hormone imbalance?
Many physicians treat individual hormone conditions. At True You, Dr. Hoff takes a whole-system approach — thyroid, sex hormones, insulin, and cortisol together — so the plan addresses how they interact.
How long does it take to rebalance hormones?
Some people feel better within weeks of starting treatment; for others it takes a few months of adjustments. We recheck labs and symptoms to fine-tune along the way.
Do I need to come into the office?
Your first visit and lab work are in person at our Fredericksburg office. Many follow-up visits after that can be virtual.
Can hormone imbalance affect sleep?
Yes. Changes in progesterone, estrogen, testosterone, cortisol, and thyroid hormone can all disrupt sleep — and poor sleep makes hormone balance worse. We work on both.
Can hormone imbalance happen at a young age?
Yes. Thyroid problems, PCOS, insulin resistance, chronic stress, and low testosterone can all affect people in their 20s and 30s. Age-based ranges shouldn’t be the reason symptoms are dismissed.
How do I get started?
Book an initial appointment. We’ll talk through your symptoms and goals and order the testing that fits your situation.
